AN ACT

 

SB 5523

 

Relating to the financial administration of the health-related licensing boards; and declaring an emergency.

 

Be It Enacted by the People of the State of Oregon:

 

          SECTION 1. To carry out the provisions of ORS 291.055 (1)(e), the following new fees, approved by the Oregon Department of Administrative Services, for the Board of Cosmetology are approved as follows:

 

           (1)     Practitioner certification-

                    original.............................................    $   50

           (2)     Practitioner certification-

                    renewal............................................    $   50

           (3)     Facility license renewal...................    $   50

           (4)     Independent contractor-

                    original.............................................    $   50

           (5)     Independent contractor-

                    renewal............................................    $   50

           (6)     CID exam fee..................................    $   50

           (7)     Temporary facility permit.............    $   50

           (8)     NSF assessment...............................    $   25

 

 

          SECTION 2. To carry out the provisions of ORS 291.055 (1)(e), the following fee increases, approved by the Oregon Department of Administrative Services, for the health-related licensing boards are approved as follows:

 

           (1)     Oregon Board of Dentistry:

           (a)     Dentist application..........................    $ 305

           (b)    Dental hygienist application...........    $ 140

           (c)     Dentist license renewal...................    $ 210

           (d)    Dental hygienist license renewal....    $ 100

           (e)     General anesthesia permit..............    $ 140

           (f)     Deep conscious sedation permit.....    $   75

           (g)     Minimal conscious sedation

                    permit..............................................    $   75

           (h)    Nitrous oxide permit.......................    $   40

           (i)      Certification of licensure................    $   20

           (j)     Late renewal fee, 11 - 30 days.......    $   50

           (k)    Late renewal fee, 31 - 60 days.......    $ 100

           (L)    Late renewal fee, 61 days-1 year...    $ 150

           (m)   Late renewal fee, 1 - 4 years..........    $ 250

           (n)    Licensure without further exam,

                    dentist and dental hygienist...........    $ 750

           (o)     Dental faculty application..............    $ 305

           (p)    Dental faculty license renewal.......    $ 210

           (q)    Dental assisting instructor

          SECTION 3. This 2001 Act being necessary for the immediate preservation of the public peace, health and safety, an emergency is declared to exist, and this 2001 Act takes effect July 1, 2001.
