  After line 8 of the printed bill, insert:
  ' (a) Provide information on:
  ' (A) Eligibility requirements and coverage provided by
publicly funded prescription drug benefit programs administered
by the Department of Human Services; and
  ' (B) The process for applying to receive publicly funded
prescription drug benefits;'.
  In line 9, delete '(a)' and insert '(b)' and delete ' patients'
and insert 'a patient'.
  In line 10, after 'medications' insert 'if the patient is not
eligible for any publicly funded prescription drug benefit
program'.
  In line 11, delete '(b)' and insert '(c)'.
  In line 14, delete '(c)' and insert '(d)'.
  In line 16, delete '(d)' and insert '(e)'.
