  On page 4, line 6, delete 'standard risk rate' and insert '
medical and portability risk rates'.
  In line 9, delete the first 'and' and insert '. The risk
rates'.
  Delete line 11 and insert 'applicable for medically eligible
individuals or 100 percent of rates established as applicable for
portability eligible individuals.'.
