  In line 8 of the printed bill, after 'form' insert 'and
protocol'.
  In line 9, delete '(5)' and insert '(6)'.
  After line 11, insert:
  ' (4) Notwithstanding subsection (3) of this section, a school
district shall require a student who is diagnosed with a
significant illness or has had a major surgery to have a physical
examination prior to further participation in extracurricular
sports.'.
  In line 12, delete '(4) The' and insert '(5) Any'.
  In line 13, delete 'or' and insert a comma.
  In line 14, after 'practitioner' insert 'or a licensed
chiropractic physician'.
  In line 15, delete '(5)' and insert '(6)' and after ' form'
insert 'and protocol'.
  In line 16, after the period insert 'The board may also adopt
by rule penalties for school districts that violate this
section.'.
