                      SENATE AMENDMENTS TO

  On page 1 of the printed bill, line 6, delete '(a) All' and
insert 'Subject to other terms, conditions and benefits in the
plan, group' and delete 'defined' and insert 'described'.
  In line 7, before 'or' insert ', coverage' and delete '
medically necessary'.
  In line 8, delete 'education'.
  Delete lines 11 through 30 and insert:
  ' (2) As used in this section, 'diabetes self-management
program' means one program of assessment and training after
diagnosis and no more than three hours per year of assessment and
training upon a material change of condition, medication or
treatment that is provided by:
  ' (a) An education program credentialed or accredited by a
state or national entity accrediting such programs; or
  ' (b) A program provided by a physician licensed under ORS
chapter 677, a registered nurse, a nurse practitioner or a
licensed dietitian with a demonstrated expertise in diabetes.'.
  On page 3, line 27, after 'to' insert 'group'.
