  On page 1 of the printed bill, line 6, delete '8' and insert
'7'.
  On page 2, line 2, after 'abortion' insert a period and delete
the rest of the line and line 3.
  In line 35, before 'at' insert 'mailed'.
  On page 4, delete lines 2 through 18.
  In line 19, delete '9' and insert '8'.
  After line 31, insert:
  ' (3) A person required to file the report under subsection (1)
of this section shall include in the report for each abortion:
  ' (a) Whether the person who provided the information required
by section 3 of this 2003 Act provided the information as the
health care provider who performed the abortion or as the
referring health care provider.
  ' (b) Whether the person who provided the information required
by section 3 of this 2003 Act provided the information in person
or by telephone.
  ' (c) Whether the person who informed the female about the
availability of the information described in section 6 of this
2003 Act informed the female in person or by telephone.
  ' (d) Whether the female received the information described in
section 6 of this 2003 Act in person or through the secure
Internet website required by section 6 of this 2003 Act or did
not receive the information.
  ' (e) Whether a medical emergency compelled termination of the
pregnancy.'.
  In line 32, delete '(3)' and insert '(4)'.
  Delete lines 37 through 45 and insert:
  ' (c) The center may not, without the consent of the health
care provider, disclose the identity of a health care provider
who performed or attempted to perform an induced termination of
pregnancy.
  ' (d) The center may not, without the consent of the health
care provider, make information available in a manner that
permits a person to identify a reporting health care provider.'.
  On page 5, delete lines 1 through 4.
  In line 7, delete '10' and insert '9'.
  In line 15, delete '11' and insert '10' and delete '8 ' and
insert '7'.
  In line 17, delete '12' and insert '11'.
  In line 20, delete '13' and insert '12'.
  In line 22, delete '12' and insert '11'.
