AN ACT

Relating to state financial administration; and declaring an emergency.

 

Be It Enacted by the People of the State of Oregon:

 

          SECTION 1. For the purpose of carrying out the provisions of ORS 291.055 (1)(e), the following new or increased fees, adopted by the Department of Human Services and approved by the Oregon Department of Administrative Services, are approved:

 

      (1)    Health services, public

              health, Office of the State

              Public Health Officer,

              health care facilities that

              hire international medical

              graduates.........................................      $         2,000

      (2)    Health services vital records:

      (a)    Birth, death, marriage,

              divorce - 1st copy............................      $              20

      (b)   Birth, death, marriage,

              divorce - 2nd copy..........................      $              15

      (c)    Expedite fee.....................................      $           5.50

      (d)   Amendments...................................      $              30

      (e)    Sealed file fee..................................      $              20

      (f)    Delayed filing..................................      $              30

      (g)    Research request for

              death certificates:

      (A)   Fewer than 100 copies....................      $              20

      (B)   100+ listing with certificate

              numbers...........................................      $              10

      (C)   100+ listing without

              certificate numbers.........................      $              15

      (h)   Nonsufficient funds

              check charge...................................      $              25

      (i)     Replacement for death and

              birth certificates after

              correction........................................      $              20

      (j)    Verification.....................................      $                8

      (k)   ALERT immunization

              registry data....................................      $              10

 

          SECTION 2. This 2005 Act being necessary for the immediate preservation of the public peace, health and safety, an emergency is declared to exist, and this 2005 Act takes effect on July 1, 2005.
