AN ACT
HB 5171
Relating to state financial administration; and declaring an emergency.
Be It Enacted by the People of the State of
Oregon:
SECTION 1. For the purpose of carrying out the provisions of ORS 291.055 (1)(e), the following new or increased fees, adopted by the Occupational Therapy Licensing Board and approved by the Oregon Department of Administrative Services, are approved:
(1) Initial occupational
therapist
license............................... $85/year
(2) Renewal of occupational
therapist
license............................... $100/year
(3) Initial occupational
therapist
license, out of state.......... $120
(4) Initial occupational therapy
assistant license, out of state........... $80
SECTION 2. For the purpose of carrying out the provisions of ORS 291.055 (1)(e), the following new or increased fees, adopted by the State Mortuary and Cemetery Board and approved by the Oregon Department of Administrative Services, are approved:
(1) Funeral establishment
license
renewal................................ $350/year
(2) Immediate disposition
company
license renewal................ $350/year
(3) Crematory license renewal............ $100/year
plus
$2/cremation
(4) Cemetery license renewal............... $4/interment
(5) Licensed facility location
change.............................................. $250
(6) Initial facility application............... $150
(7) Initial reciprocal funeral
service
practitioner or
embalmer
application..................... $160
(8) Initial preneed salesperson
license............................................... $75
(9) Individual funeral service
practitioner
or embalmer
license............................................... $80/year
(10) Combination funeral service
practitioner
and embalmer
license............................................... $160/year
SECTION 3. For the purpose of carrying out the provisions of ORS 291.055 (1)(e), the following new or increased fees, adopted by the State Board of Pharmacy and approved by the Oregon Department of Administrative Services, are approved:
(1) Remote dispensing machine
license............................................... $100/year
(2) Remote dispensing machine,
controlled substance registration... $25/year
SECTION 4. This 2005 Act being necessary for the immediate preservation of the public peace, health and safety, an emergency is declared to exist, and this 2005 Act takes effect July 1, 2005.
