  On page 2 of the printed A-engrossed bill, after line 27,
insert:
  ' (c) A youth is found responsible except for insanity under
ORS 419C.411.'.
  In line 28, delete '(c)' and insert '(d)'.
  On page 3, after line 3, insert:
  ' (f) The name and contact information of the individual to
contact for further information about the notice;'.
  In line 4, delete '(f)' and insert '(g)'.
  In line 6, delete '(g)' and insert '(h)'.
  In line 7, delete '(h)' and insert '(i)'.
  After line 20, insert:
  ' (c) A notice required by subsection (2)(c) of this section
shall contain substantially the following statement: 'This notice
is to inform you that a disposition order has been entered in a
case involving a student who attends your school about whom a
previous notice was sent. The disposition order finds the student
to be responsible except for insanity under ORS 419C.411 for the
act alleged in the petition filed with the juvenile court. The
case should not be discussed with the student.' ' .
  In line 21, delete '(c)' and insert '(d)' and delete ' (2)(c)'
and insert '(2)(d)'.
  In line 33, delete 'or'.
  In line 35, delete the period and insert '; or
  ' (f) The juvenile court enters a disposition order finding the
youth responsible except for insanity under ORS 419C.411.'.
  On page 5, line 34, after 'youth' insert 'is not enrolled in
the school administrator's school but'.
  After line 43, insert:
  ' (e) The principal of the private school if the youth attends
a private school.
  ' (f) The appropriate school administrator if the youth attends
a school in another school district.'.
  On page 7, after line 19, insert:
  ' (E) The name and contact information of the individual to
contact for further information about the notice;'.
  In line 20, delete '(E)' and insert '(F)'.
  In line 21, delete '(F)' and insert '(G)'.
  After line 40, insert:
  ' (F) The name and contact information of the individual to
contact for further information about the notice;'.
  In line 41, delete '(F)' and insert '(G)'.
  In line 44, delete '(G)' and insert '(H)'.
  On page 8, after line 24, insert:
  ' (f) The name and contact information of the individual to
