insert 'therapist'.
  After line 16, insert:
  ' (o) Audiologist.
  ' (p) Speech language pathologist.'.
  On page 3, after line 42, insert:
  ' (t) Physical, speech or occupational therapist.
  ' (u) Audiologist.
  ' (v) Speech language pathologist.'.
