  In line 8 of the printed bill, after 'certificate' insert ' ,
signed and dated within six months preceding the date of
application,'.
  In line 10, after 'certificate' insert ', signed and dated
within six months preceding the date of application,'.
  In line 11, delete 'and'.
  After line 11, insert:
  ' (2) The state-issued licensing number of the licensed
physician, certified nurse practitioner, licensed physician
assistant or licensed optometrist who signed the certificate
described in subsection (1) of this section; and'.
  In line 12, delete '(2)' and insert '(3)'.
